Physical Forms
Elder High School is a member of the Ohio High School Athletic Association (OHSAA). As a member, we abide by organization rules. Student participation in sports is explained in the OHSAA 2009-10 Student-Athliete Eligibilty Guide which you can access through this link.
Planning to participate in sports at Elder? If so, a physical examination is required by both Elder and the Ohio High School Athletic Association.
The Elder Athletic Department has made arrangements with several physicians for the evening of Monday, July 20, 2009. Physicals will be available “free” to prospective athletes this evening, starting at 7:00 p.m. in the The Schaeper Center. Fall athletes must have Physicals, Emergency Medical insurance, and OHSAA Authorization (Hippa release) forms in by August 1st; winter athletes by November 1; and spring athletes by February 1st.
Athletes should complete their Emergency Medical form, the front side of the Physical form and the OHSAA Authorization and bring with their insurance payment to the physical. Checks for insurance should be made out to “Elder High School”. Failure to turn in will result in athletes not being able to participate in tryouts.

At Elder, our medical coverage for the 2009/2010 school year is through Mutual of Omaha. This is a supplemental insurance that will cover medical bills your insurance does not cover. The payment scale may be found below, and as in the past, the insurance is mandatory. If you do have any bills or questions, contact Ray Bachus at Elder.
