Physical Forms

Elder High School is a member of the Ohio High School Athletic Association (OHSAA).  As a member, we abide by organization rules.  Student participation in sports is explained in the OHSAA 2009-10 Student-Athliete Eligibilty Guide which you can access through this link.

Planning to participate in sports at Elder?  If so, a physical examination is required by both Elder and the Ohio High School Athletic Association.

Athletes should complete their Emergency Medical form, the front side of the Physical form and the OHSAA Authorization and bring with their insurance payment to the physical.  Checks for insurance should be made out to “Elder High School”.  Failure to turn in will result in athletes not being able to participate in tryouts.

At Elder, our medical coverage for the 2011/2012 school year is through National Union with Maskin as program & claims manager.  This is a supplemental insurance that will cover medical bills your insurance does not cover.  The payment scale may be found below, and as in the past, the insurance is mandatory.  If you do have any bills or questions, contact Ray Bachus at Elder.
